The endometrium is normally the inner lining of the uterus that grows during the cycle under the effect of female hormones. In the absence of pregnancy, the endometrium detaches from the uterus causing menstruation. This tubal reflux of menstrual blood is physiological, however in some patients, the endometrium which arrives in the abdominal cavity will, instead of being cleaned successfully take root and survive, thus creating endometriosis.<\/p>\n<h2>What are the main symptoms of endometriosis ?<\/h2>\n<p>Sometimes endometriosis is discovered incidentally during a laparoscopy performed for another reason. Otherwise, infertility and pain are the two major symptoms of endometriosis. They can sometimes be enough to soothe pain and reverse the disease at least temporarily. However, in cases of associated infertility, personalised care of the couple is offered (surgery, insemination, IVF &#8230;).<\/div>\n\t\t\t\t   <\/div><div class=\"boc_list_item boc_animate_when_almost_visible boc_top-to-bottom \" >\n\t\t\t\t\t<span class=\"li_icon normal \" style=\"\"><i class=\"icon icon-angle-right-circle\" style=\"\"><\/i><\/span>\n\t\t\t\t\t<div class=\"boc_list_item_text normal\"><strong>Surgery\u00a0<\/strong>is particularly indicated in patients with painful endometriosis. A very large majority of patients noted a positive effect on pain after surgery.<\/div>\n\t\t\t\t   <\/div>[vc_column_text css_animation=&#8221;boc_animate_when_almost_visible boc_bottom-to-top&#8221; el_class=&#8221;yesjustify&#8221; css=&#8221;.vc_custom_1511949150541{margin-top: 15px !important;margin-bottom: 0px !important;}&#8221;]<\/p>\n<h2>Key points of endometriosis surgery<\/h2>\n<p><strong>Surgical treatment of endometriosis can sometimes be very delicate and absolutely must be performed by a qualified surgeon. Indeed, an incomplete initial or poorly performed surgery may cause persistent or worsening symptoms and greatly complicates any new surgery.<\/strong><\/p>\n<p>A careful clinical examination and imaging examinations (pelvic ultrasound and M.R.I) are useful before surgery, to establish a precise mapping of the lesions.<\/p>\n<p>The goal of surgery is to remove most of the endometriosis lesions while leaving the affected and surrounding organs intact.<\/p>\n<p>Moreover, as this surgery often involves women of childbearing age, it is crucial to preserve the reproductive potential of the patient as far as this is at all possible. For example, in cases of ovarian endometriotic cysts, removal of the cyst can cause a significant decrease in the ovarian reserve, and therefore we must be very careful with this procedure, especially if the patient wishes to conceive.<\/p>\n<h2>Digestive endometriosis<\/h2>\n<p>When endometriosis reaches the wall of the colon or rectum, there are two possible approaches:[\/vc_column_text]<div class=\"boc_list_item boc_animate_when_almost_visible boc_top-to-bottom \" >\n\t\t\t\t\t<span class=\"li_icon normal \" style=\"\"><i class=\"icon icon-search\" style=\"\"><\/i><\/span>\n\t\t\t\t\t<div class=\"boc_list_item_text normal\">The first option is used when the endometriosis only affects a superficial part of the wall of the intestine and the lesion can be removed by shaving the lining of the affected organ: this is called the \u201cshaving\u201d technique. The complication rate is low and the results in terms of pain and fertility are good.<\/div>\n\t\t\t\t   <\/div><div class=\"boc_list_item boc_animate_when_almost_visible boc_top-to-bottom \" >\n\t\t\t\t\t<span class=\"li_icon normal \" style=\"\"><i class=\"icon icon-search\" style=\"\"><\/i><\/span>\n\t\t\t\t\t<div class=\"boc_list_item_text normal\">The second option is used when the lesion is embedded deep into the wall of the organ. The shaving technique would almost certainly leave\u00a0some disease behind. In this case a more radical treatment (see images opposite) is selected: after removing the diseased part of the colon or rectum, an anastomosis is performed, that is to say, a new connection between the two parts of the intestine that are healthy. This procedure is only proposed to selected patients because it can certainly offer a significant improvement in the patient\u2019s quality of life but, it also comes with a high risk of complications.<\/div>\n\t\t\t\t   <\/div>[vc_column_text css_animation=&#8221;boc_animate_when_almost_visible boc_bottom-to-top&#8221; el_class=&#8221;yesjustify&#8221; css=&#8221;.vc_custom_1511949198679{margin-top: 15px !important;margin-bottom: 0px !important;}&#8221;]<\/p>\n<h2>Urinary endometriosis<\/h2>\n<p>Regarding endometriosis reaching the bladder wall, either the shaving technique can be performed, or a partial opening of the bladder can be made to remove the entire lesion. The bladder is then sutured, and generally heals very well.<\/p>\n<p>If the lesion is very close to the ureters (the small pipes leading urine from the kidneys to the bladder), the procedure carries a higher risk of urinary complications.
